Acknowledgment of Risks
I, the undersigned participant, acknowledge that my participation in the Lorain County JVS Homecoming involves certain inherent risks, including but not limited to physical injury, property damage, and other potential hazards. I understand and voluntarily accept these risks.
Release and Waiver of Liability: In consideration of being allowed to participate in the Lorain County JVS Homecoming, I, on behalf of myself and my heirs, executors, administrators, and assigns, hereby release, waive, and discharge the Lorain County Joint Vocational School District, its officers, directors, employees, agents, volunteers, and all other parties involved in organizing and conducting the event from any and all claims, demands, actions, or causes of action arising out of or relating to any injury, loss, or damage that I may suffer while participating in the event.
Indemnification: I agree to indemnify and hold harmless the Lorain County Joint Vocational School District from any claims or liabilities arising out of my participation in the event, including any claims brought by third parties.
Medical Treatment Authorization: In the event of an emergency, I authorize the Lorain County Joint Vocational School District or its representatives to seek and consent to any medical treatment deemed necessary for my well-being, including the administration of first aid and transportation to a medical facility.
Photographic Release: I grant permission for the Lorain County Joint Vocational School District to use photographs, videos, or other likenesses of me taken during the event for promotional or educational purposes.
Agreement to Terms: I have carefully read and understood this Hold Harmless Agreement and Liability Waiver. I acknowledge that I am voluntarily participating in the Lorain County JVS Homecoming with full knowledge of the risks involved, and I agree to comply with all event rules and guidelines.
